When you request an appointment, you may share your name, phone number, email address, preferred timing, or the type of care you need.
We use those details to respond to your request.
The form is meant for scheduling. Please do not include medical details or urgent concerns. For urgent needs, call the office at (973) 595-8700.
Dynamic Dental Arts links to separate Dentrix systems for new patient forms or patient account access.
Those portals are not part of this public website. Information entered there is handled under the portal provider's privacy practices.
Outside Services
Some links or features rely on services operated by other companies. Their privacy practices apply when you use those services.
Appointment requests are delivered through Formspree. The information you submit passes through Formspree so the office can receive it.
Online bill payments open a separate SwipeSimple page. Card details are entered there, not on the public Dynamic Dental Arts website.
CareCredit provides a separate financing website. Google Maps opens directions or location details. Google Fonts supplies the typefaces used by this site.

Information on this site is general information about the practice. It does not replace advice from a dentist or create a patient relationship.
